This application information is for domestic sanctions only (those events which do not include participation by foreign athletes, coaches and/or judges).

To apply by mail or fax:

Sanction requests must be received in the Member Services office a minimum of 30 days prior to the start of the event. Late fees apply for any sanction received less than 30 days prior to the event and additional late fees apply when request is one week or less.

Professional member number and password
Valid credit card

Apply for Sanction online – use this link to apply for a domestic event sanction
Complete all required information
Enter payment information
Submit the application
Your sanction request will be entered into the database system.

Note: The sanction approval form and meet report form will be printed in the USA Gymnastics office. You will not be able to print these documents online. Sanctions are printed in the USA Gymnastics office 2 times a week. The forms are sent to the club address listed on the sanction request form. Be sure to complete the online application at least 7 days prior to your event so you can receive your materials in time for the event. We are not able to provide a certificate of sanction if you apply online the day of your meet.

Request for Sanction on the Same Day as Event:
You must fax your request (using the form above) with credit card payment information to 317-692-5212. Special RUSH processing fees of $100.00 will apply. USA Gymnastics may refuse or may not be able to process same-day sanction requests. We are not able to provide a certificate of sanction if you apply online the day of your meet.
